JOB DATES: 9/22/01, 10/23/01 & 10/24/01
SUMMARY OF WORK;
The interior surfaces of the hvac: systems servicing the first, second, third, fourth and fftlt
floors at Syms were cleaned. On completion of source removal, "Envirocon", was
applied for control of bacteria. All work was performed in accordance with NADCA
General Specifications for the Cleaning of Commercial lHeating, Ventilating & Air
Conditioning Systems. All areas were visually inspected on completion of work. The
cleaning was performed at night and all areas were unoccupied during the project.
All the ventilation systems contained a build up of dust, especially heavy, in the return air
ducts. All of the ducts were constructed with fiberglass-lined sheet metal. The build up
in the ducts was to be expected based on the age of the systems and previous

There was no visible

IAQ PROGRAM RECOMMENDATIONS: Continue to document quarterly filter
