To: Mike Nolan, Project Manager Warren & Panzer Engineers, P.C.

From: Leonard Atteloney, Director of Laboratory Services Bureau of Environmental Compliance

Date: September 16,2002

Topic: Final Visual Inspection Reports ********************************************************************************* Enclosed are the “Final Visual Inspection Report” that were found to be unacceptable for the following reasons:

a) The use of “Yes or No” in the Visual Inspection Checklist section. A “No” would seem to indicate that debris was observed in that location but the Technician circled “Pass.” b) On some reports the Technician checked all the locations in the Visual Inspection Checklist section. The technicians need to be specific for the locations where cleanup was done. c) Reports that indicate “Fail” must be submitted with reports that showed “Pass” due to additional cleanup activities.

Please revise these reports and resubmit as soon possible.

c: M. Gilsenan, Asst. Commissioner BEC J. Terhune, Managing Principal, Warren & Panzer
