All sections of the application have been completed, although in some areas the answer to a question is shown by underlining one of the possible answers (the hard copy is clearer). I am also attaching the consent form, modeled on the ATSDR/NYC study, and the cover letter (in which an expedited review, typically 3 days, is requested). As soon as the application, cover letter, and consent form are finalized, they can be packaged with the scope of work (received), building/apartment questionnaire (received), and CVs for Carol and Nancy (to be received), and submitted to the IRB.
