Attached is a memo I received from Zoe Ann Campbell, concerning the establishment of a WTC Health Registry by the NYC Mark D. Hoffer General Counsel Department of Health and Mental Hygiene. This is a voluntary program intended to collect health data on people who lived, worked or attended Bureau of Legal Affairs school near the WTC site, and were exposed to smoke, dust or debris from Tel (718)595-6555 the collapse of the World Trade Center. Fax (718) 595-6543 www.ci.nyc.ny.u8/dep The first page of Zoe Ann’s memo indicates the categories of persons who would be covered by the Registry. If you fall into one of these categories and wish to participate, a WTC Registry Form is also attached and should be completed and returned to Herb Roth by September 30*^.
