• The Greater New York Hospital Association has established a Patient Locator System Database
with some information on people affected in the form of a searchable database. You may access

Death Certificate Information
Families may go to the Family Assistance Center at Pier 94 for help in securing a death certificate.
Please call 646-710-6245 or go to www.nvc.gov and click on WTC Death Affidavit Forms/Information.
